TITLE

HCU40 valve failures

DETAILS OF INCIDENT

All four HCU40 machines in the department received a field service update on internal valves. A few months later, 3 of the machines have leaked. The new valves failed causing the machines to be unusable. One machine failed in theatre without warning and electrical noises could be heard. The other two machines failed on defrosting. All machines failed in the space of a week.
