TITLE

Centrimag battery module failure

DETAILS OF INCIDENT

"We had an emergency situation in the cardiac catheter lab, being stented, arrested, CPR performed.

ECMO requested. During pre ECMO checks and as cannulation began, console alarm sounded saying: battery module failure".

DETAILS OF INJURY TO PATIENT

None

TYPE OF DEVICE/MANUFACTURER

ECMO console was swapped with the back up console already available on our trolley whilst cannulation proceeded. VA ECMO was instigated with minor delay due to the wait whilst the console swap was being made.

ACTION TAKEN

Datix completed and the company was informed with a request to investigate the faulty console.
